Annual Membership Fee

Wellness  Plan: $449

Includes:

• Two Periodic Exams per year

• Two Routine Cleanings per year (not including SRP or periodontal maintenance )

• One Emergency Exam per year

• Check-up X-Rays (4 bitewings  and either 1 full mouth series or 1 panoramic)

• 15% discount on all other dental procedures*

Periodontal Wellness Plan:  $749

 Includes:

• Periodic Exams (2 per year)

• Routine Perio Maintenance (4 per year excluding SRP and adjunct procedures )

• Limited exam ( 1 per year)

• Check-up X-Rays (4 BWs and 1 FMX or 1PAN)

• 15% off all other dental procedures*
